ABF Youth Cardiac Screening Program is being held
by the Anthony Bates Foundation (ABF) in your community. As many as 1,000 young people and their families are expected to
take advantage of the free hearth health screenings. The Foundation has organized and hosted events in Arizona, California,
Kansas, Missouri, New York, and Washington,. Hundreds of additional student athletes will receive screenings from our Cardiac
Screening friends across the United States to enhance this effort to save young lives from
the tragedy of Sudden Cardiac Arrest. A portion of the proceeds from our cardiac
screening donations go into the community as AED (automatic external defibrillator) donations to schools and another portion
goes directly to ABF. The ABF contributions will further the mission to raise
awareness of the leading killer of young athletes in this country (HCM, Hypertrophic Cardiomyopathy).
Reap
the value of helping prevent an unnecessary young person’s life; become a sponsor of our Youth Cardiac Screening Program. The Foundation anticipates screening
up to 1,000 youth & their family members in our next event. Our initial press
date will be 6 weeks prior to the event.
We
would like your help in one or more of these areas:

Bronze Sponsors. Table space is available on the day
of the event for economy tier is $100 to share your Heart Healthy products, business coupons, and support the mission to protect
young lives. Fees will assist in the purchase of AEDs for the community.

Food Sponsors: $150 tier fee to receive recognition
during the event. In-kind donations are accepted.
Feed our hundreds of Volunteers during any of our Cardiac Screening Day events.

T-shirts. $250 tier fee and the cost provide t-shirts
for the event volunteers and/or participants. Including Bronze table space, signage placement, and advertisement will be included
in this sponsorship support.

Silver Sponsor:
Table space in a premiere location and signage during the event for $500 support
·
Gold Sponsor: Table
space in a premiere location, signage during the event at one of the event stations,
and direct advertisement on the ABF promotional materials for $1000 support
·
AED Sponsor for purchase: With a $2,000 tier support,
an AED will be purchased in your name. Permanently display your support by Protecting
Youth and Community members with your donation. Also receive Gold Sponsorship
status for this event.
