Currently, the Anthony Bates Foundation (ABF) has two end-of-life Siemens Cypress ultrasound / echo machines. These machines do not allow ABF to capture the ultrasound / echo images into our storage. We need to add "new/used" ultrasound equipment and eventually replace the older ultrasound equipment to improve the quality of the Anthony Bates Foundation research program and add two additional ultrasound machines to increase the screening capacity.

NO PURCHASE IS NECESSARY TO ENTER OR WIN. A PURCHASE DOES NOT INCREASE THE CHANCES OF WINNING.
1. Eligibility: This Campaign is open only to those who sign up at the Anthony Bates Foundation website (www.AnthonyBates.org/sos-contest) and who are 16 years or older as of the date of entry. The Campaign is only open to legal residents of the United State of America, and is void where prohibited by law. Employees of the Anthony Bates Foundation, its affiliates, subsidiaries, advertising and promotion agencies, and suppliers, (collectively the “Employees”), and immediate family members and/or those living in the same household of Employees are not eligible to participate in the Campaign. The Campaign is subject to all applicable federal, state, and local laws and regulations. Void where prohibited.
2. Agreement to Rules: By participating, the Contestant (from here on referred to as “You”) agrees to be fully and unconditionally bound by these Rules, and You represent and warrant that You meet the eligibility requirements. In addition, You agree to accept the decisions of the Anthony Bates Foundation, regarding allocation of all prizes, as final and binding as it relates to the content of this Campaign.
3. Campaign Period: Entries will be accepted online starting on November 1, 2020, 12 AM Arizona Time Zone and ending extended to December 31, 2020, 11:59 PM, Arizona Time Zone. All online entries must be received by December 31, 2020, 11:59 PM, Arizona Time Zone.
4. How to Enter: The Campaign must be entered by submitting the online entry form provided at www.AnthonyBates.org/sos-contest. You, the entrant, must fulfill all Campaign requirements, as specified, to be eligible to win a prize. Entries that are incomplete or do not adhere to the rules or specifications may be disqualified at the sole discretion of the Anthony Bates Foundation. Optional verbiage to include: You may enter only once. You must provide the information requested. You may not enter more times than indicated by using multiple email addresses, identities, or devices in an attempt to circumvent the rules. If You use fraudulent methods or otherwise attempt to circumvent the rules, Your submission may be removed from eligibility at the sole discretion of the Anthony Bates Foundation.
5. Prizes: The Grand Prize Winner of the Campaign (the “Winner”) will receive (2) Round Trip airline vouchers on American Airlines or Southwest Airlines $800 retail value. Actual/appraised value may differ at time of prize award and travel selected dates. The specifics of the prize shall be solely determined by the Anthony Bates Foundation. No cash or other prize substitution shall be permitted except at the Anthony Bates Foundation’s discretion. The prize is nontransferable. Any and all prize-related expenses, including, without limitation, any and all federal, state, and/or local taxes, shall be the sole responsibility of Winner. No substitution of prize or transfer/assignment of prize to others or request for the cash equivalent by Winner is permitted. Acceptance of prize constitutes permission for the Anthony Bates Foundation to use Winner’s name, likeness, and entry for purposes of advertising and trade without further compensation, unless prohibited by law.
6. Odds: The odds of winning depend on the number of eligible entries received.

To qualify for additional prizes, each Campaign participate will be invited to create Your “Peer-2-Peer Fundraising Team.” Then You as the “Team Leader” will invite your friends, family, acquaintances and business to donation to the ‘campaign’ and all donations will benefit the Anthony Bates Foundation (ABF) to replace old ultrasound machines and support the 2021/2022 Cardiac Screening events scheduled in Arizona, Colorado, Kansas and New England.

8. Grand Prize Winner Selection and Notification: Winner will be selected by a random drawing of the “Peregrine Falcon Group” Campaign members under the supervision of the Anthony Bates Foundation. Winner will be notified by email and telephone within five (5) days following selection of Winner. The Anthony Bates Foundation shall have no liability for Winner’s failure to receive notices due to spam, junk e-mail or other security settings or for Winner’s provision of incorrect or otherwise non-functioning contact information. If Winner cannot be contacted, is ineligible, fails to claim the prize within 10 days from the time award notification was sent, or fails to timely return a completed and executed declaration and release as required, the prize may be forfeited and an alternate Winner selected. Receipt by Winner of the prize offered in this Campaign is conditioned upon compliance with any and all federal, state, and local laws and regulations. Terms & Conditions: the Anthony Bates Foundation reserves the right, in its sole discretion, to cancel, terminate, modify or suspend the Campaign should virus, bug, non-authorized human intervention, fraud, or other cause beyond the Anthony Bates Foundation’s control corrupt or affect the administration, security, fairness, or proper conduct of the Campaign. In such case, the Anthony Bates Foundation may select the Winner from all eligible entries received prior to and/or after (if appropriate) the action taken by the Anthony Bates Foundation. The Anthony Bates Foundation reserves the right, in its sole discretion, to disqualify any individual who tampers or attempts to tamper with the entry process or the operation of the Campaign or website or violates these Terms & Conditions. - As a participation gift to all people that donate over $25, OR operate a Peer-2-Peer Fundraising campaign in the ABF S.O.S Campaign that receives over $24 in donations will receive a S.O.S Campaign T-Shirt OR an ABF Totebag from ABF.
- ABF will have weekly drawings of gift-cards ($25 value) and various ABF Swag for all team members that have donated in the week of $25 or more. Donations must be received by Tuesday at 11:59 PM to qualify for the Wednesday afternoon (3:00 PM) drawing. Winners will be announced on Social media and to the team leader.
- Donations (Cash, Check and On-Line Credit Card are accepted), and Pledges (one-time, monthly, or annual are accepted) received through the Campaign will count towards Campaign Prize Selection Values as outlined below.
- Once your Peer-2-Peer Campaign is created by You as the “Team Leader”, ABF recommends emailing your friends & family about your commitment to assist in the ABF S.O.S Campaign, posting on Social Media (every week during the campaign period starting – 11/1 through 12/31) to alert your friends and supporters of your status in the campaign.
- There are four categories of the S.O.S. Campaign for selective prizes in each category.
